‘Read about Me’ is a simple, effective new way to assist people with a dementia or cognitive impairment who are receiving medical care.
The toolkit was created by Cardiff and Vale Health Board and encourages patients to offer person-centred information.
This allows staff to have a better understanding of the patient without the need for repetition of their personal story. This helps to maintain continuity of care for them throughout their journey.
