Our Care Hubs provide community-based support for older people, including those living with dementia, helping them to stay active and connected. Through personalised care, social activities and meaningful engagement, the service supports wellbeing while also providing opportunities for unpaid carers to take a break.
We have trained carers who can assist those who:
- are living with dementia,
- experiencing age-related illness,
- have a physical disability and/or
- require support with aspects of daily living.
Locations
Care Hubs are based in:
- Plasmawr Road Fairwater,
- Minehead Road (Llanrumney), and
- Grand Avenue (Ely).
What we offer
We base our service around you by getting to know you, supporting cognition, providing group and individual activities, adapting these to cater to all abilities and providing support with personal care if needed. We focus on strengths and maintaining independence.
Our trained staff support people to stay independent, active and engaged.
We provide:
- personalised care and support,
- group activities designed to suit different needs,
- support with daily living,
- opportunities to build confidence and social connections, and
- respite for carers.
We focus on individual strengths and support independence wherever possible.
Activities and community connections
We work with a range of partners to offer meaningful activities, including:
- music therapy,
- reminiscence sessions, and
- intergenerational activities.
Supporting people with dementia
All Care Hub services support people affected by dementia.
Staff create safe, welcoming environments where people can:
- connect with others,
- stay engaged, and
- enjoy meaningful activities.
We are committed to helping people maintain their wellbeing and quality of life.
Times and meals
Care Hubs usually run from 8:30am to 4:30pm. For a small charge we provide a three-course lunch and snacks and drinks throughout the day, meals can be adapted to suit dietary requirements.
Transport
We can arrange transport to and from the Care Hub.
You can also travel independently if you prefer.
