The First Point of Contact Team provides information, advice, and assistance to adults, helping them to remain independent in their own homes and stay connected to their communities.
A key focus of their work is prevention and early intervention, ensuring you can access the right support at the right time.
The FPOC Team takes the time to have “What Matters” conversations, focusing on:
- what is important to you
- your strengths
- the outcomes you would like to achieve
- understanding your circumstances
- identifying your personal goals
- exploring the support available to help you live independently, safely, and well as possible.
FPOC work closely with a wide range of partners to promote independence and wellbeing including:
- health services
- third sector organisations
- housing providers
- community-based support services.
Wherever possible FPOC will explore community-based and preventative solutions first to help you achieve your goals and maintain your independence. This may include:
- support such as aids and adaptations
- falls prevention services
- benefits and welfare rights advice
- community groups
- wellbeing services
- and other local resources that can enhance your quality of life.
FPOC recognise that the best outcomes are often achieved by building on your existing strengths, support networks, and connections within your community. If your needs cannot be met through community-based support alone, FPOC will refer to the most appropriate Adult Social Care team for further assessment and consideration of any ongoing care and support needs.
FPOC aim is to provide a timely, person-centred response that helps you find solutions, maximise your independence, and achieve the outcomes that matter most to you.
