Our Community Engagement Team supports people aged 60 and over to stay independent and connected to their communities.
We work with people who may be socially isolated, have a physical disability, or live with an age-related condition, such as dementia.
Support is short term and focused on helping people build confidence and regain independence.
If the team identifies that longer-term support is needed, we may refer you to services such as social work or occupational therapy.
How we support you
Firstly, we will meet with you in your home to understand what matters to you and what you would like to achieve.
Then, we can help you to:
- reconnect with your local community,
- build confidence and skills, and
- safely access local areas, groups and activities.
Support is personalised and based on your individual needs and goals.
The Community Engagement Team help people access activities and opportunities in their local area.
This can include:
- joining social groups,
- taking part in community activities, and
- developing new interests.
The aim is to help you feel more connected and confident in everyday life.
Digital engagement
We can offer a digital service to help you get and use technology at home, with up to six support sessions to build your confidence and help you reconnect with local groups and activities.
